Principles of PACC

  • Physical Activity (PA) is an evidence based prescription for chronic disease.

  • Peer learning through group discussion, teach back, and hands on physical fitness activities (low-moderate intensity)

  • Therapeutic fitness modalities must be at the center of patient care.

  • Develop strategic action plans to improve the delivery and outcomes of PA healthcare services

Goals of PACC

  • Improve health service delivery

  • Reduce cost of chronic illness

  • Reduce hospital admissions

  • Reduce ER visits

  • Improve patient satisfaction

  • Improve adherence to care plan

The objective of PACC is to train CHWs in the presentation and implementation of therapeutic fitness strategies which emphasize self-care and promotion of healthy lifestyle behaviors to reduce chronic disease and its accompanying comorbidities impacting communities with the highest health disparities.
